# Registration form

> Choose which questions buyers answer at checkout, whether each is required, and the order they appear in.

The **Registration Form** page lets you customize the questions buyers answer during checkout. Open it from **Registration Form** in the event menu.

Answers are collected per attendee. You can review them later; for example, on the [RSVP](/events/rsvp) page each attendee has a **Form Responses** view.

## Two kinds of questions

The questions are shown in two tables, each with the columns **Question**, **Required** and **Status**.

* **Preset Questions**: common details PLVR already knows how to collect.
* **Custom Questions**: questions you write yourself.

If a table has nothing in it yet, it says "No preset questions added" or "No custom questions added".

## Add a preset question

Under **Quick add**, select a button for the detail you want. Buttons only appear for questions you have not added yet, and may include **Gender**, **Nationality**, **Phone Number**, **Date of Birth** and **Identity Document**. You see "\<question> question added", and the new question is set to required.

## Create a custom question

**1. Select Custom question**

The button is at the right of the Quick add row. A panel titled **Create Custom Question** slides in.

**2. Fill in the question**

* **Question\***: required.
* **Description**: optional help text.
* **Answer Type\***: **Text**, **Number**, **Single choice** or **Multiple choice**.
* **Options\***: shown for single and multiple choice. Add one option per line with **Add Option**. At least one option is required, and you can remove options once you have more than one.
* **Required**: switch on if buyers must answer.

**3. Save**

Select **Save**. You see "Question saved" and the question appears under Custom Questions.

## Manage existing questions

* **Required.** Use the switch in a row to mark a question required or optional. You see "Marked as required" or "Marked as optional".
* **Status.** Choose **Active** or **Inactive** from the dropdown. Inactive questions stay in your list but are no longer asked. You see "Question is now active" or "Question is now inactive".
* **Order.** Drag a row by its handle to reorder. Presets and custom questions are ordered separately, within their own table. You see "Question order updated".

> [!NOTE]
>
> Name and email are always collected. Their Required and Status controls are locked, and selecting them shows "Cannot modify required status for system fields".

> [!TIP]
>
> This page covers the questions asked when buying. If your event uses PLVR Tap, a separate Personalization Form works the same way. See [Personalization form](/tap/personalization-form).
