# Personalization form

> Choose which attendee-identity questions are asked before a wristband is bound.

The **Personalization Form** controls the identity questions attendees answer when they personalize or claim a ticket, before their wristband is bound to it. It's listed in the event sidebar and only appears for events with PLVR Tap enabled.

It works like the [registration form](/events/registration-form), but it is a separate form with its own questions.

## Add questions

### Quick add

Click one of the **Quick add** buttons to add a standard question. Each is added as required, and the button disappears once added:

* **Gender**
* **Nationality**
* **Phone Number**
* **Date of Birth**
* **Identity Document**

### Custom questions

**1. Click Custom question**

A **Create Custom Question** panel opens.

**2. Describe the question**

* **Question** (required)
* **Description** (optional)
* **Answer Type** (required): **Text**, **Number**, **Single choice**, or **Multiple choice**
* **Options** (for the two choice types): add at least one option, using **Add Option**
* **Required** switch

**3. Save**

The question appears under **Custom Questions**.

## Manage your questions

Questions are listed in two tables, **Preset Questions** and **Custom Questions**. Each shows **Question**, **Required**, and **Status**.

* **Required:** use the switch to make a question required or optional.
* **Status:** set a question to **Active** or **Inactive**. Inactive questions are not asked.
* **Order:** drag the handle on the left of a row to reorder questions within its table.

> Attendees' names and email are always collected as fixed fields, so they aren't listed here.
